Testing of a specimen will be initiated with the following:
1. A completed test requisition form with patient signature
2. Guardian's Date of Birth
3. The indication(s) for testing and/or appropriate Test Codes are required for billing and results interpretation.

Corporate accounts are available for physician/clinical offices, referral laboratories, and hospitals. Establishing a corporate account provides the following benefits:

A check/DD/Wire Transfer payable to "Eurofins Clinical Genetics India Pvt. ltd., Bengaluru" for the agreed to price of the test.
